Commonwealth Home Support Program

The Commonwealth Home Support Program (CHSP) provides entry-level care services for people who need occasional or short-term assistance to maximise independence and remain living in their own home.

This is often the first level of support offered in the aged care system.

How to apply

If you're seeking government assistance for aged care services, you'll need to apply for an in-person assessment with My Aged Care. Or, contact us to organise private services.

• 1. Check your eligibility

  To check your eligibility for an aged care assessment, call My Aged Care on 1800 200 422.
  Have your Medicare number, address and contact details ready.

• 2. Apply for an assessment

  If you're eligible, call 1800 200 422 or complete My Aged Care's online application (allow 15 to 20 minutes).

• 3. Prepare for your assessment

  Visit the My Aged Care website for a full list of things you should prepare before your assessment.

• 4. Have your assessment

  Your aged care assessment will take place at home or in hospital. Consider having a support person such as a family member or carer with you.

• 5. Get your assessment outcome

  You’ll receive an assessment letter that will show your approved aged care services and funding level and provide an individual support plan.

• 6. Organise your aged care services

  Once you’re approved, call us on 08 8724 1251 with your referral code to organise your services with Boandik or we’ll contact you if you nominated Boandik as your provider.
